Better Outcomes for Members with Heart Disease, Diabetes
We’re more than just an insurance plan, we’re a health plan. We’ve learned that chronic diseases, such as heart disease and diabetes, are among the most prevalent, costly and preventable of all health problems. That’s why we offer a unique approach to Disease Management that improves member health while improving your clients’ bottom line.
We use claims analysis and our award-winning health assessment to identify members with high-risk conditions. By reaching out to these members, we alert them of their risk potential and engage them in health improvement strategies. To ensure that members get the customized care they need, we stratify members from low to high severity. This helps us provide targeted, actionable intervention tailored to fit each member’s needs.
Last year, HealthPartners engaged 13,589 members whom we identified as having coronary artery disease (CAD), diabetes or both CAD and diabetes. Of those, most members chose to enroll in programs:
- CAD: 69 percent
- Diabetes: 74 percent
- CAD and diabetes: 79 percent
Plus, member satisfaction with HealthPartners Disease Management programs ranges from 85-95 percent. With HealthPartners, your clients and their employees get better care and better coverage with HealthPartners.

CDHP Check Reimbursement Update
To improve operational efficiency in the Consumer-directed Health Plan (CDHP) Operations Department, we’re changing check reimbursement timing for FSAs and HRAs. Reimbursements will now be processed on Tuesdays and checks will be mailed to members on Wednesdays, which is one day earlier than the current process. Electronic fund transfers (EFTs) will still be sent to the bank on Fridays.
Here’s what your clients may want to know:
- Claims will continue to be processed within one to two business days.
- Reimbursements will be run for all claims processed through Monday of each week.
- Claims received on Mondays will likely be reimbursed the following week.
- Claims received on Thursdays are likely to make the Tuesday mailing.
- Claims received on Friday are likely to make the Tuesday mailing if processing volumes are at a normal level.
- All processing turnaround is dependant on claims volume.
- Some participants may be used to sending in claims over the weekend or even on Mondays and expecting to get reimbursed that same week, this will no longer be the case with the new process.
We are also changing the explanation of benefits (EOB) for flexible spending accounts (FSAs) and health reimbursement accounts (HRAs). We are no longer attaching the reimbursement check directly to the EOB. The check will be sent in an envelope with the EOB, but will not be attached. If a member has elected to have funds automatically deposited they will no longer receive a check that has non-negotiable written across it. Members will now receive and EOB with the words "DRAFT" on it to indicate that the reimbursement is being transferred to their account.
